Registered Nurse (RN)

Job Description

We are seeking an experienced Registered Nurse (RN) to provide home health care services. The ideal candidate will have a strong background in CMS/OASIS Home Health programs with a focus on delivering high-quality patient care in a home setting.

Shift Details

• Schedule: Monday through Friday
• Hours: 8:00 AM to 4:30 PM for regular shifts
• Weekend Requirement: On-call coverage required, averaging one weekend per month
• Overnight availability: 1-2 days a week from 4:30 PM to 8:00 AM
• Holiday Requirement: Rotating holiday coverage will be required

Responsibilities

• Provide comprehensive nursing care to patients in their homes
• Conduct patient assessments, prepare care plans, and provide education to patients and their families
• Utilize CMS and OASIS documentation guidelines to maintain accurate patient records
• Coordinate care with other healthcare professionals and community resources

Qualifications

• Minimum of 2 years of experience in CMS/OASIS Home Health program
• Preferred: 5 or more years of home health experience
• Licensed as a Registered Nurse in the state
• Must have reliable transportation and will receive mileage reimbursement

All candidates must complete necessary credentials prior to starting and will be paid on a weekly schedule.
