Physical Therapist (PT)

Job Description

The Physical Therapist will be responsible for screening, evaluating, and rehabilitating patients experiencing physical functioning disorders and/or requiring wound care. The therapist will develop and implement appropriate plans of care in accordance with regulatory and clinical practice standards.

• Utilize standardized assessments and evidence-based practices to guide clinical interventions.
• Administer various rehabilitation procedures, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Assess patient progress and adjust treatment plans as necessary, ensuring documentation adheres to regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluations.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Maintain effective communication with supervisory personnel and other health team members regarding patient progress, barriers, and treatment plans.
• Compile comprehensive discharge summaries of services rendered in compliance with regulatory and clinical requirements.
• Document all necessary information in the electronic documentation system and maintain patient records in accordance with company policy.
• Accurately record treatment minutes for services provided in alignment with all regulatory and clinical guidelines.
• Participate in required meetings such as patient care conferences and rehabilitation meetings.
• Engage in facility and company mandated training sessions, in-services, and conferences.
• Obtain necessary durable medical equipment or supplies following regulatory and clinical practice standards.
• Supervise and support physical therapist assistants, rehabilitation aides, and students.
• Adhere to APTA and state practice acts, code of conduct, and ethics in all professional dealings.

Qualifications

• Minimum of 1 year of experience as a Physical Therapist.
• Experience in Long Term Care (LTC) or rehabilitation settings is required.
• Active Oklahoma State License is mandatory.
