Physical Therapist (PT)

Job Description

The Physical Therapist (PT) is responsible for evaluating the physical therapy needs of assigned patients and establishing goals in collaboration with patients. This role involves planning, developing, and implementing individualized physical therapy programs aimed at improving patient mobility and independent function. The PT provides developmentally appropriate care to patients of all ages, from pediatrics to geriatrics, and effectively interacts with patients, physicians, and other healthcare team members.

Key Responsibilities

• Assess, plan, treat, evaluate, and implement a plan of care for all clients seen.
• Demonstrate understanding of the admission process, treatment costs, coding, financial support, and appropriate billing practices.
• Communicate and interact with patients, families, physicians, and staff in a manner consistent with the organization’s core values.
• Complete documentation of all patient care activities promptly in the electronic medical record system.
• Assist with transferring and positioning patients as required.

Qualifications

• Minimum of 1 year of experience in Home Health as a Physical Therapist.
• Valid driver’s license and car insurance.
• COVID vaccination is required; declinations will not be approved.
• OASIS and start of care experience is strongly preferred.
• Current BLS certification.
• Active Illinois state license at the time of submission.
