LTC/SNF Registered Nurse (RN)

We are seeking a dedicated LTC/SNF Registered Nurse (RN) to join our nursing team. This position offers the opportunity to support and care for patients in a long-term care and skilled nursing facility.

Job Details

• Shift: Days and Nights (6am-2:30pm, 2pm-10:30pm, 6pm-6:30am, 6-6:30pm)
• Shift Length: 8 or 12 hours
• Guaranteed Hours: 36 hours per week

Responsibilities

• Provide holistic nursing care to residents with a focus on dementia and geriatrics.
• Collaborate with the healthcare team to develop and implement resident care plans.
• Oversee and assist nursing staff during assigned shifts, ensuring compliance with protocols.
• Ensure adequate staffing levels and assist in training new team members.
• Participate in patient assessments and monitor vital signs as required.
• Communicate effectively with residents, families, and team members.

Qualifications

• Must possess a valid Registered Nurse (RN) license.
• Minimum of 2 years of nursing experience in a long-term care or skilled nursing facility is required.
• BLS Certification is required.
• Ability to work every other weekend and every other holiday commitment.

Additional Information

• Facility consists of 51 beds with nurse-to-resident ratios of 2 nurses during the day and 1 overnight nurse.
• Electronic Medical Record (EMR) system used is PCC.
• Orientation includes 2 hours with the leadership team and 1-2 shifts on the floor with experienced staff.
• Local rates apply within proximity, with no charge differential applicable.
