Skilled Nursing Facility Position

Overview

We are seeking dedicated professionals to join our team at a skilled nursing facility that offers post-hospital, short-term rehabilitation, as well as long-term and respite care services. Our facility is recognized for its commitment to quality care, having earned specialized certifications in heart failure management.

Schedule

• Full-time position, 40 hours per week
• Monday to Friday schedule
• Weekend rotation required

Responsibilities

• Provide high-quality care to residents according to individualized care plans
• Assist with daily activities, ensuring the comfort and well-being of residents
• Collaborate with a multidisciplinary team to enhance resident outcomes
• Monitor and document changes in residents’ health status
• Support rehabilitation and therapy programs within the facility

Qualifications

• Relevant experience in skilled nursing or rehabilitation settings preferred
• Strong interpersonal and communication skills
• Ability to work effectively in a team-oriented environment
