Skilled Nursing Care Provider

We are seeking dedicated professionals to join our team in a skilled nursing environment that offers post-hospital, short-term rehabilitation, as well as long-term and respite care. Our facility proudly accepts Medicare, Medicaid, and most private insurances, and is committed to serving the medical and rehabilitation needs of individuals recovering from illness or injury.

Key Responsibilities

• Provide high-quality nursing care to residents in accordance with established nursing protocols and standards.
• Assist in the development and implementation of individualized care plans based on the needs of each resident.
• Monitor and record vital signs, and report any changes in a resident’s condition to the supervising nurse.
• Collaborate with a multidisciplinary team to enhance patient outcomes and ensure a positive rehabilitation experience.
• Support residents’ daily living activities, ensuring comfort and dignity in care.

Qualifications

• Current and valid nursing license.
• Experience in a skilled nursing setting is preferred but not mandatory.
• Strong interpersonal and communication skills.
• Ability to work collaboratively within a team environment.
• Commitment to ensuring the highest quality of care for residents.

Work Schedule

This position involves a full-time schedule of 40 hours per week, Monday through Friday, with a weekend rotation.
