Registered Nurse (RN)

We are seeking a qualified Registered Nurse (RN) with essential experience in nursing homes and a keen understanding of Patient Care Coordination (PCC). The ideal candidate will possess strong clinical skills and demonstrate a commitment to delivering high-quality patient care.

Role Responsibilities

• Provide thorough and compassionate care to residents in a nursing home setting.
• Coordinate patient care through effective communication with healthcare team members.
• Document patient assessments, care plans, and progress effectively.
• Administer medications and treatments as prescribed and monitor patients for any changes in condition.
• Educate residents and their families regarding health conditions and care options.

Qualifications

• Current and valid RN license.
• Experience in a nursing home or similar environment is mandatory.
• Strong understanding of Patient Care Coordination (PCC).
