Job Description

We are seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) for a contract position focusing on case management within a diverse acute care setting. This role involves collaboration with healthcare teams to ensure effective patient care and coordination.

Key Responsibilities

• Conduct assessments and develop discharge plans for patients in acute care.
• Provide care coordination and support for patients transitioning from hospital to home.
• Collaborate with multidisciplinary teams, including nursing, physicians, and other specialists.
• Float as needed between various units within the healthcare facility.
• Participate in interdisciplinary meetings to discuss patient care and outcomes.
• Ensure compliance with all regulations and quality standards.
• Address social, emotional, and economic factors that may affect patient care.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Active Washington state license as a Licensed Clinical Social Worker (LCSW).
• A minimum of 1 year of acute care experience is required.
• Previous travel experience is required.
• Experience with EPIC electronic medical records is mandatory; AIDA EMR experience is preferred.
• Strong preference for candidates with diverse case management experience, including Med Surg, Length of Stay (LLOS), and Emergency Department (ED).
• Certifications: Basic Life Support (BLS) certification required.

Work Schedule

• Shift: Day shift (5×8 hours, 07:30 – 16:00).
• Weekends: Required every other weekend.
• Guaranteed hours: 72 hours within a 2-week period.

Additional Information

• Floating required between different areas of the hospital as needed.
• Parking costs will be covered by the facility.
• All requests for time off (RTO) must be submitted at the time of application.
