Skilled Nursing Facility (SNF) Nurse

Job Description

We are seeking a dedicated and experienced nurse to join our team in a Skilled Nursing Facility environment. The ideal candidate will possess a minimum of 2 years of experience in a SNF setting and demonstrate a commitment to providing high-quality patient care.

Shifts

Shifts will consist of either 5×8 or 4×10-hour schedules with a requirement to work every weekend.

Qualifications

• Minimum of 2 years of experience in a Skilled Nursing Facility (SNF) is preferred.
• Current and valid nursing license.
• Strong communication skills and the ability to work effectively within a team.
• Compassionate and patient-centered approach to care.
• Ability to manage and prioritize multiple tasks in a fast-paced environment.

Responsibilities

• Provide direct patient care and support to residents in the Skilled Nursing Facility.
• Monitor and document patients’ medical conditions, vital signs, and medications.
• Collaborate with healthcare team members to deliver optimal care.
• Educate patients and their families on health management and care plans.
• Participate in ongoing training and professional development activities.
