Social Worker for Pre/Post Liver Transplant Patients

We are seeking a dedicated and compassionate Social Worker specializing in the care of pre and post liver transplant patients. This position entails conducting psychosocial evaluations and providing essential support throughout the transplant process.

Responsibilities

• Complete psychosocial evaluations for pre and post liver transplant patients.
• Manage a case load of approximately 10 new patients weekly.
• Verify caregivers over the phone to ensure suitable support systems are in place.
• Provide evaluations for inpatients as needed based on departmental requirements.
• Collaborate with multidisciplinary teams to enhance patient care and outcomes.
• Document and maintain accurate records of patient interactions and evaluations.

Schedule

This position offers a Monday to Friday schedule, with a 5-day workweek, consisting of 8-hour days. No weekends are required.

Qualifications

• Active LICSW (Licensed Independent Clinical Social Worker) license is required.
• Experience working with transplant patients and understanding the specific psychosocial needs associated with organ transplantation is preferred.
• Strong communication and interpersonal skills to effectively engage with patients and families.
• Ability to work independently and as part of a team in a fast-paced environment.
• Flexibility to adapt to varying patient care needs and departmental demands.

Work Environment

This role is primarily stationed in an outpatient setting, but may require participation in inpatient evaluations based on departmental needs.
