Social Worker

Job Description

We are seeking a dedicated Social Worker to join our multidisciplinary treatment team. In this role, you will be pivotal in providing direct care and support to patients and their families, making impactful decisions that enhance the well-being of those you serve.

Responsibilities

• Actively participate in the health care team and provide direct and indirect care to patients, families, and legal guardians.
• Deliver clinical social work services in collaboration with physicians, nurse practitioners, and direct care staff.
• Conduct biopsychosocial assessments and contribute to multidisciplinary treatment planning, case management, individual and family therapy, and discharge strategies.
• Assist with insurance utilization review and coordinate care with patients, families, community agencies, and programs.
• Collaborate with the multidisciplinary treatment team to develop, implement, and document appropriate treatment goals and interventions, informed by patient strengths and presenting psychiatric concerns.

Qualifications

• Master’s degree in a clinical field is required.
• Current LCSW license or eligibility for LMHC is mandatory; prior experience in inpatient behavioral health or acute care settings is preferred.
• Child & Adolescent Needs & Strengths (CANS) Certification must be obtained within 30 days of hire.
• Advanced knowledge of clinical behavioral health treatment and family systems for patients with psychiatric illnesses is essential.
• Experience and proficiency with Electronic Health Records, including Medsphere/Vista, is preferred.
• Understanding of community support systems available for post-discharge patients is crucial.
• Bilingual English/Spanish skills are preferred but not required.

Work Schedule

This is a full-time position requiring 40 hours per week during the day shift. Weekends are not required.
