LTC/SNF LPN

Job Overview

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our Long-Term Care/Skilled Nursing Facility (LTC/SNF) team. Our ideal candidate should have a strong commitment to providing high-quality patient care while fostering a supportive and nurturing environment.

Key Responsibilities

• Administer medications and treatments as prescribed by medical staff.
• Monitor patient health and report any changes to the nursing team.
• Assist with daily living activities and ensure patient comfort.
• Document patient care and progress in compliance with facility standards.
• Collaborate with interdisciplinary team members to develop individualized care plans.
• Provide education and support to patients and their families.

Shift Details

This position requires working a 12-hour shift from 6:00 AM to 6:00 PM, including every other weekend and holiday. A total of 36 hours per week is guaranteed.

Qualifications

• Minimum of 2 years of experience in a similar role.
• Current BLS Certification is required.
