Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)

Job Description

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to join our long-term care team. The ideal candidate will have a passion for providing high-quality patient care, with a focus on assisting patients with various medical needs, particularly those with dementia.

Key Responsibilities

• Provide nursing care to patients in a long-term care setting.
• Assist with daily living activities and ensure patient comfort and safety.
• Monitor patient vital signs and report any significant changes to the healthcare team.
• Administer medications and treatments as prescribed by licensed healthcare providers.
• Collaborate with interdisciplinary teams to develop and implement personalized care plans.
• Maintain accurate patient records and documentation.
• Float to different units as needed to meet staffing needs.

Qualifications

• Must possess a current and valid New York nursing license.
• Basic Life Support (BLS) certification required.
• Minimum of 1 year of nursing experience in a long-term care environment.
• Strong communication and interpersonal skills.
• Ability to work flexible hours, including day and evening shifts, and every other weekend.

Work Schedule

This position requires working 36 hours per week, with the possibility of floating between units as needed. Shifts include either 6am-6pm or 10am-10pm. Regular weekend shifts are required every other weekend.

Patient Population

The primary focus of care will be on patients with dementia and other related diagnoses.
