LTC/SNF Registered Nurse (RN)

Job Description

We are seeking a skilled LTC/SNF Registered Nurse (RN) to join our team. The ideal candidate will play a critical role in providing high-quality care to residents in a long-term care or skilled nursing facility.

Shift Information

• Shift Length: 12 hours
• Guaranteed Hours: 36 hours per week
• Schedule: 6:00 AM – 6:30 PM; every third weekend and every other holiday

Responsibilities

• Deliver patient care in accordance with established standards and regulations.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Monitor and assess patients’ health conditions, providing updates to the nursing team and physicians as necessary.
• Maintain accurate patient records and documentation.
• Educate patients and their families on care plans and health management strategies.
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.

Qualifications

• Must possess a valid Registered Nurse (RN) license.
• BLS Certification required.
• Minimum of 2 years of nursing experience, preferably in LTC/SNF settings.
• Exceptional communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

Additional Information

• Workweek: Sunday through Saturday.
• Position availability is immediate, until a permanent hire is made.
• This position requires flexibility to accommodate varying needs within the unit.
• Hours may vary between 24 to 36 hours per week, based on unit requirements.
