Social Worker

We are seeking a dedicated and qualified Social Worker to join our team. The ideal candidate will possess strong clinical skills and a passion for helping patients in an outpatient setting.

Position Overview

The Social Worker will work primarily in a Dialysis Center and will be required to manage cases of patients with renal failure. This role will not involve clinical involvement, focusing instead on providing support and resources to patients and their families.

Key Responsibilities

• Provide psychosocial support to patients receiving dialysis treatment.
• Assist patients in navigating healthcare services and community resources.
• Conduct assessments to determine patients’ needs and appropriate interventions.
• Collaborate with healthcare teams to optimize patient care and outcomes.
• Travel or conduct tele visits to two additional locations as needed.

Qualifications

• Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW) required.
• Current licensure for practice in the relevant state.
• Minimum of 1 year of experience in a social work role, preferably in outpatient settings.
• BLS Certification required.
• No prior travel experience is necessary.

Work Schedule

This position requires working from 8:00 AM to 5:00 PM, with flexibility for travel or tele visits to sister locations.

Additional Information

• Orientation provided for one week with a preceptor available throughout the assignment.
• No weekend or holiday requirements.
• Business casual professional attire is expected for meetings.
