Licensed Clinical Social Worker (LCSW)

Job Summary

The Licensed Clinical Social Worker (LCSW) will provide essential social work services in an outpatient setting. This position involves collaborating with a multidisciplinary team to support patients facing various challenges, primarily focusing on renal failure management within a Dialysis Center.

Key Responsibilities

• Conduct comprehensive assessments and develop care plans tailored to patient needs.
• Assist patients in navigating healthcare systems and accessing resources.
• Provide support and counseling to patients and their families.
• Maintain detailed documentation of patient interactions and progress.
• Collaborate with healthcare professionals to ensure holistic patient care.
• Travel or conduct tele-visits to sister locations as needed.

Qualifications

• Master’s Degree in Social Work (MSW) or equivalent required.
• Valid North Dakota License as a Clinical Social Worker (LCSW).
• BLS Certification required.
• Minimum of 1 year of post-graduate experience in a similar role.
• Previous travel experience is not required.

Work Schedule

This position offers a standard work schedule from 8:00 AM to 5:00 PM, with the requirement to float between locations with prior notice. Weekend and holiday work are not required.

Dress Code

The dress code for this role is business casual professional for meetings.

Orientation

The successful candidate will participate in one week of unit/department specific orientation, with a preceptor available throughout the assignment.

Additional Information

This role requires travel or tele-visits to multiple locations within the region. Candidates should be prepared to manage a varied patient load based on location, averaging around 95 patients per month.
