Job Description

Join a dedicated team at a skilled nursing facility that provides comprehensive post-hospital rehabilitation as well as long-term and respite care services. Our focus is on meeting the medical and rehabilitation needs of individuals who are recovering from illness or injury.

Key Responsibilities

• Provide high-quality patient care and support rehabilitation efforts.
• Assist patients with daily living activities and ensure their comfort.
• Collaborate with healthcare staff to coordinate care plans and treatments.
• Monitor patient progress and report any changes in condition to the medical team.
• Maintain accurate patient records in compliance with facility policies.
• Participate in weekend rotation as required to ensure continuous care.

Job Details

• Schedule: Monday to Friday with weekend rotation
• Hours: 40 hours per week
• Salary Placement Fee: 10%

Qualifications

• Relevant experience in a skilled nursing or rehabilitation environment is preferred.
• Compassionate and patient-centered approach to care.
• Ability to work collaboratively within a multidisciplinary team.
• Strong communication and interpersonal skills.
