Job Description

We are looking for dedicated professionals to join our team in a skilled nursing facility that specializes in post-hospital rehabilitation, long-term, and respite care. This facility offers a supportive environment for both short-term and long-term care residents.

Key Responsibilities

• Provide high-quality patient care in accordance with established protocols and guidelines.
• Assist residents with daily living activities and ensure a comfortable environment.
• Monitor patient health and report any significant changes to the healthcare team.
• Collaborate effectively with multidisciplinary teams to ensure optimal patient outcomes.
• Participate in care planning and implementation of individualized care plans.

Schedule

The position requires full-time commitment with a standard schedule of 40 hours per week, Monday through Friday, along with a weekend rotation.
