Acute Care Case Manager

We are seeking a skilled Acute Care Case Manager to join our team, with an immediate start date available. The ideal candidate will have a strong background in case management within an acute care setting.

Job Responsibilities

• Manage discharge planning and case management for patients.
• Ensure effective communication and collaboration among healthcare team members.
• Maintain patient care quality and safety standards.
• Participate in departmental meetings and training sessions as required.

Qualifications

• Minimum of 2 years of experience in Acute Care Case Management or Discharge Planning.
• Must possess a valid CA state license (pending licenses accepted, but must be licensed upon start date).
• Experience in non-acute settings, such as Behavioral Health, does not apply.

Work Schedule

• Weekend availability is required to meet department needs (every other weekend).
• RTO restrictions should not exceed 7 days.
