Skilled Nursing Facility (SNF) Rehabilitation Specialist

Job Description

We are seeking a dedicated and skilled individual to join our team in a rehabilitation role within a Skilled Nursing Facility (SNF). The ideal candidate will have a passion for providing high-quality care and support to patients in their recovery process.

Responsibilities

• Provide rehabilitation services to patients in a skilled nursing environment.
• Assess patient needs and develop individualized treatment plans.
• Monitor patient progress and make necessary adjustments to treatment plans.
• Collaborate with multidisciplinary teams to ensure comprehensive care.
• Maintain accurate patient records and documentation in line with regulatory requirements.

Qualifications

• Minimum of 1 year of experience in a Skilled Nursing Facility (SNF).
• Familiarity with NetHealth (Rehab Optima) software is preferred.
• Current and active California license is required.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
