LTC/SNF LPN

We are seeking a dedicated LTC/SNF LPN to join our team. The ideal candidate will be responsible for providing excellent care to elderly patients with multiple diagnoses in a long-term care and skilled nursing environment.

Job Details

• Shift Length: 12 hours
• Guaranteed Hours: 36 hours per week
• Shift Notes: 6:00 AM – 6:30 PM; every other weekend and holiday required

Key Responsibilities

• Administer medications and treatments as prescribed.
• Monitor patient health and document changes.
• Assist in daily living activities to ensure patient comfort and well-being.
• Collaborate with healthcare teams to develop and implement patient care plans.
• Provide education and support to patients and their families.

Qualifications

• Minimum of 1 year of experience as a licensed practical nurse (LPN).
• Current BLS Certification required.

Facility Information

• Beds: Current census is 42.
• Patient Ratios: Day shift is typically 1:21, with a maximum of 1:42.
• Electronic Medical Record (EMR): PCC.
• Weekend and Holiday Requirement: Every other weekend and holiday.
• Orientation: 1 day of location-specific orientation followed by up to 3 days on the floor with staff.
