Job Description

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality care to residents of a long-term care and skilled nursing facility. The successful candidate will play a critical role in supporting patient health and welfare.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and record patient health information and vital signs.
• Assist with daily living activities, ensuring comfort and safety of residents.
• Collaborate with nursing staff and healthcare professionals to assess patient needs and implement care plans.
• Maintain accurate and detailed patient records.
• Educate patients and their families on health management and disease prevention.

Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
• Experience in long-term care or skilled nursing preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of a team.
• Commitment to providing quality care and support to residents.

Onboarding Requirements

• Completion of a comprehensive health history assessment.
• Verification of nursing license.
• Basic immunization records including MMR, Hepatitis B, Influenza, TB test, Varicella, and Tdap (optional).
• Professional documentation including driver’s license/state ID, references, resume, and background checks.
• Completion of competency testing, including core competencies and skills checklist.
