Physical Therapist

We are seeking a qualified Physical Therapist to join our team, with responsibilities primarily in Skilled Nursing Facility (SNF) and Home Health settings across the region. This position will also provide support in neighboring areas when needed.

Key Responsibilities:

• Supervise and collaborate with two Physical Therapy Assistants (PTAs).
• Maintain productivity expectations of 80% for SNF services.
• Deliver therapy services following established guidelines and standards of care.
• Utilize software systems including SNF Net Health and Home Health HCHB effectively.
• Work closely with the onsite Therapy Supervisor for clinical guidance and support.

Schedule Details:

• Work schedule varies; primarily day shifts.
• Guaranteed hours: 40 hours per week.
• Flexible scheduling to accommodate various needs in SNF and Home Health settings.

Qualifications:

• New graduates are welcome to apply.
• Possess a valid physical therapy license.

Additional Information:

• This role may involve travel between multiple locations within the region.
• Expected to adhere to a 36-hour minimum for twelve-hour shifts or a 40-hour standard for eight to ten-hour shifts.
• Work week is Sunday through Saturday.
