Physical Therapist (PT) – Skilled Nursing Facility (SNF)

Job Description

We are seeking a dedicated Physical Therapist (PT) to join our team in a Skilled Nursing Facility (SNF). The ideal candidate will have experience in long-term care settings and a strong commitment to patient rehabilitation.

Key Responsibilities

• Provide high-quality physical therapy services to patients in accordance with their individual care plans.
• Work collaboratively with other healthcare professionals to ensure comprehensive patient care.
• Assess patient needs and monitor progress to adjust treatment plans accordingly.

Shift Details

• Shift: 8-hour Days (08:00 – 16:30)
• Ratios: 1:5
• Weekend Coverage: Not required
• On-Call Duty: Not required

Qualifications

• Experience Required: Minimum of 1 year of experience in Skilled Nursing Facility (SNF) and/or Long-Term Care (LTC) settings.
• First-Timers: Not accepted.
• Certifications Required: Basic Life Support (BCLS) certification.
• Licensure: Pending license acceptance is allowed.
• Transportation: Travelers are expected to have their own transportation.

Additional Information

• Vaccination Requirement: Primary series COVID vaccination in addition to one booster is mandatory.
• Guaranteed Hours: The facility can call off up to one shift per pay period.
• Parking: Free parking available on-site.
• Mileage Reimbursement: Provided at the IRS Standard rate.
• Charting System: Net Solutions.
