Case Manager
Position Information
The role of a Case Manager is essential in providing support and coordination for patients in an acute freestanding rehabilitation setting.
Shift and Hours
- Shift: 8:00 AM – 4:30 PM, Monday to Friday
- Weekly Guaranteed Hours: 40
- Weekend Hours: 0
- Call Requirements: 0
Qualifications
- Current licensure as a Registered Nurse, Social Worker, or other healthcare professional such as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist, along with relevant case management experience.
- 2-3 years of experience preferred.
- Certifications in case management are preferred, such as:
- Commission for Case Manager Certification (CCM)
- Association of Rehabilitation Nurses (ARN) certification
- American Case Management Association (ACM)
- Board Certification in Case Management (RN-BC)
- Effective oral and written communication skills in English; proficiency in additional languages is a plus.
- Basic computer skills in Excel, Word, Outlook, etc. are required.
Key Responsibilities
- Collaborate with nursing, therapy teams, and medical professionals to support patient care.
- Engage in family conferences and utilize reviews to optimize patient outcomes.
- Advocate for patients while balancing resource management and healthcare realities.
- Participate in eligibility assessments for acute rehabilitation criteria.

- Capable of handling challenging discussions professionally with families and patients regarding insurance and healthcare realities.
