Licensed Clinical Social Worker (LCSW)

Job Description

We are seeking a skilled Licensed Clinical Social Worker (LCSW) to join our Case Management team. The ideal candidate will bring acute care experience and a strong background in discharge planning and care coordination.

Key Responsibilities

• Provide case management services across diverse units including Medical Surgical, Length of Stay (LLOS), and Emergency Department (ED).
• Coordinate care and discharge plans for patients in an acute care setting.
• Assist in managing patients’ needs and resources effectively.
• Collaborate with healthcare teams to facilitate patient care and transitions.
• Float between different units as necessary within the facility.

Requirements

• MUST have valid Washington state licensure.
• Minimum of 1 year of acute care experience required.
• Epic experience is required; AIDA EMR experience is preferred.
• Must be willing to work weekends, with a rotation of every other weekend.
• Candidates must have strong communication and organizational skills.
• BLS certification is required.
• Previous travel experience is necessary; first-time travelers will not be considered.

Contract Details

• Contract Length: 13 weeks
• Shift: 5×8 Days (07:30 – 16:00)
• Guaranteed Hours: 72 hours per 2-week period
