LPN (Licensed Practical Nurse)

Job Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in a Long-Term Care (LTC) and Skilled Nursing Facility (SNF) environment. The ideal candidate will possess a strong background in providing quality care to residents with varying needs.

Shift Information

• Evening and Night Shifts Available
• Shift Length: 8 or 12 Hours
• Guaranteed Hours: 40 hours per week
• Weekend Requirements: Yes
• Holiday Requirements: Yes

Responsibilities

The LPN will be responsible for:

• Providing direct patient care and support to residents
• Administering medications and treatments as prescribed
• Collaborating with healthcare team members to develop and implement care plans
• Monitoring residents’ vital signs and reporting any changes to the nursing staff
• Assisting with daily living activities and ensuring the comfort of residents

Qualifications

• Minimum of 1 year of experience in a nursing role
• Current BLS Certification
• Strong communication Skills
• Ability to work flexible hours, including weekends and holidays

Patient Care Environment

The facility is equipped with:

• 30 Beds
• Patient Care Ratios: 30 to 1 with 2 medication aides or 15 to 1
• Electronic Medical Record (EMR) System: Matrix point of care
• Common Patient Diagnoses: Mild dementia, bariatric care
