Position Overview

The Social Worker will provide essential support in care coordination and case management, focusing on a variety of patient populations. This role involves collaboration with interdisciplinary teams to ensure effective discharge planning and delivery of social services.

Job Details

• Shift: 10-hour days
• Guaranteed Hours: 40 hours per week
• Work Schedule: Days, 0830-1700
• Weekend Requirement: Rotating
• Dress Code: Business Casual
• Orientation: Onsite and via Google Meet Online

Responsibilities

• Conduct discharge planning and facilitate care coordination to ensure seamless transitions for patients.
• Engage in care management and provide support to patients and their families, addressing social, emotional, and financial challenges.
• Handle caseloads of up to 40 patients/consults per day, prioritizing effectively based on patient needs.
• Maintain accurate documentation within the Electronic Medical Record (EMR) system, specifically using Epic.

Qualifications

• Education: BSW (Bachelor of Social Work) required for office role or MSW (Master of Social Work) required for inpatient settings.
• Experience: Minimum of 2 years in social work, with preference for inpatient and emergency department experience.
• Licensure: NE CSW license required.
• Certifications: BLS Certification from AHA required.

Additional Information

• Artificial nails are prohibited in all departments providing patient care.
• A valid driver’s license is required at the time of submission to confirm distance eligibility.
