Registered Nurse (RN) – Home Health

Position Overview:

The role involves providing direct patient care in a home health setting, focusing on case management and patient assessments. The RN will be responsible for delivering quality health services to patients while coordinating care and ensuring adherence to medical protocols.

Key Responsibilities:

• Conduct patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed by healthcare providers.
• Provide wound care and manage ongoing patient health conditions.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Document patient progress and maintain accurate health records using the OASIS system.
• Educate patients and their families on health management and wellness strategies.

Position Details:

• Department: Home Health
• Number of Openings: 1
• Shift: Days (8:00 AM – 4:30 PM)
• Weekly Guaranteed Hours: 36
• Weekend Hours: Monday to Friday only; no weekends required
• Call Requirements: None

Qualifications:

• Required Certifications: BLS (Basic Life Support)
• Experience: Minimum of 1 year in home care setting preferred.
• Preferred Skills: Wound care experience and familiarity with OASIS and EPIC documentation systems.

Patient Care Focus:

This position involves a patient ratio of 5-6 patients daily. The RN will work in a dynamic home health environment, requiring strong organizational and communication skills to effectively manage patient care.

• Mileage: Reimbursed at the current IRS standard rate.
• Dress Code: Black scrubs are required.
