Physical Therapist (PT) – Home Health

Assignment Details

The Home Health Physical Therapist plans and administers medically prescribed physical therapy treatments for patients experiencing injuries, as well as muscle, nerve, joint, and bone conditions. The aim is to restore function, alleviate pain, and prevent disability. Key responsibilities include:

• Providing education and information to patients and their caregivers.
• Participating in required discharge planning.
• Completing all necessary documentation according to policy.
• Guiding patient participation in selected tasks to restore, reinforce, and enhance their performance.

Schedule

Work hours are from 8:00 AM to 5:00 PM, Monday through Friday. Weekend work is required only for orthopedic cases, with physical therapy coverage needed every 4th or 5th weekend.

Unit Information

• Mileage reimbursed at the IRS rate.
• Expected 6 to 7 weighted visits per day.

Minimum Requirements

• At least 1 year of experience is required, with a preference for home health experience.
• Unit is open to first-time travelers.
• Experience with MatrixCare or Epic is preferred but not required.

Required Certifications

• Virginia Physical Therapy State License Required – can be pending.
• CPR certification is required.
