Registered Nurse – Case Manager

We are seeking a dedicated Registered Nurse specializing in Case Management. The ideal candidate will possess strong clinical skills and experience in care coordination and discharge planning within an acute care hospital setting.

Job Responsibilities

• Perform care coordination and discharge planning for patients.
• Collaborate with interdisciplinary teams to ensure quality patient care and effective discharge processes.
• Manage the patient caseload in accordance with department needs and standards.
• Participate in weekend rotations as required by the department.

Qualifications

• Minimum of 2 years of experience as a Case Manager in an acute care hospital setting.
• A valid Registered Nurse (RN) license in the state of California is required; pending licenses will be accepted if the candidate is licensed by the start date.
• First-time travelers will be considered; clinical screening will apply.
• Ability to accommodate a maximum of 7 days for requested time off.

Work Schedule

• Day shift: 5 days a week, each shift lasting 8 hours (08:00 – 16:30).
• No block schedules available.
