Job Description

This role involves providing rehabilitation services in skilled nursing facilities (SNF) and/or intermediate care facilities (ICF). The Occupational Therapist will be responsible for the following:

• Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice.
• Develop, implement, and carry out individualized treatment plans in collaboration with supervising therapists, physicians, and interdisciplinary teams, as appropriate.
• Utilize evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life, as applicable to the discipline.
• Educate residents, families, and caregivers regarding treatment plans and patient progress.
• Complete accurate, timely, and compliant documentation, including progress notes, changes in plan of care, and discharges according to facility, state, federal, and payer requirements.
• Supervise therapy assistants, when applicable to the role.

Requirements

• Minimum of two (2) years of experience in the SNF/ICF setting preferred.
• Current Basic Life Support (BLS) certification.

Work Schedule

Generally scheduled for five (5) days per week, including one weekend day. There may be a possibility of floating to other units or facilities based on census and staffing needs.

Training

Orientation will be provided, including training on the rehabilitation department, electronic medical record (EMR) documentation systems, and facility workflows.
