Physical Therapist (PT)

Job Description

The Physical Therapist will be responsible for effectively screening, evaluating, and rehabilitating patients with physical function disorders and/or wound care. The therapist will develop appropriate plans of care following all regulatory and clinical practice standards.

Key Responsibilities

• Utilize standardized assessments and evidence-based practices to support clinical interventions.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Assess patient progress and adjust treatments in the clinical record as required by regulation and clinical practice.
• Evaluate treatment outcomes for effectiveness, modifying services as needed and making referrals based on outcomes.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Communicate effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans.
• Prepare comprehensive discharge summaries as per regulatory and clinical requirements.
• Document all regulatory and clinically required information in the electronic documentation system, maintaining accuracy in patient records in accordance with company policy.
• Record treatment minutes accurately for services provided per all regulatory and clinical requirements.
• Participate in required meetings, including patient care conferences and rehabilitation meetings.
• Engage in facility and company-required trainings, in-services, and conferences.
• Obtain necessary DME or supplies in accordance with regulatory and clinical practice requirements.
• Supervise and support physical therapist assistants, rehabilitation aides, and students.
• Adhere to the APTA and state-required practice acts, code of conduct, and ethics during professional practice.

Qualifications

• Minimum of 1 year of experience as a Physical Therapist is required.
• Long Term Care or Rehabilitation experience is required.
• Must hold a valid Oklahoma State License.
• New graduates are encouraged to apply, provided they have completed Level II clinical experiences.
