Case Manager

Job Description

The Case Manager will be responsible for assessing, evaluating, and driving the discharge plan for patients. This position involves coordinating services to ensure that patients receive the appropriate level of care. Daily responsibilities include initiating, evaluating, and re-evaluating treatment plans, monitoring patient progress and identifying any barriers to discharge, and following Medicare regulatory guidelines. The Case Manager will also be expected to monitor the length of stay for patients actively.

Key Responsibilities

• Assess and evaluate discharge plans for patients.
• Coordinate services to align with the patient’s required level of care.
• Initiate, evaluate, and re-evaluate treatment plans daily.
• Monitor patient progress and address any lack of progress.
• Identify barriers to timely discharge.
• Adhere to Medicare regulatory guidelines concerning patient choice, IMM notices, and MOONS.

Schedule

This position follows a Monday to Friday schedule with no weekend or call requirements. Floating may be required within the facility and scope of practice as needed.

Qualifications

• A minimum of 3 years of relevant experience is required.
• First-time travelers will not be considered for this position.

Compliance and Orientation

Full compliance with all pre-employment requirements is mandatory for timely start. Non-compliance may result in a postponement of up to two weeks due to the bi-weekly orientation schedule.

Travelers are expected to work on major holidays if they fall within the assignment period, with specific expectations outlined during the assignment period.

Request Time Off (RTO)

Maximum of 7 days for RTO is allowed, with no RTO requests accepted during September. Candidates are encouraged to discuss requests during the interview process.

Additional Information

Orientation hours will vary based on the length of the contract assignment.
