Registered Nurse (RN) – Case Manager

Job Description

We are seeking a dedicated Registered Nurse (RN) specializing in Case Management to join our healthcare team. This position requires an immediate start and offers a rewarding opportunity for those with acute care experience.

Key Responsibilities

• Provide comprehensive case management and discharge planning for patients.
• Maintain a patient-to-nurse ratio of 1:20 during the day shift.
• Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
• Adhere to best practices and standards in case management.
• Participate in weekend and holiday shifts as required (every other weekend and available for holidays).

Qualifications

• Minimum of 2 years of experience in acute care case management or discharge planning.
• Current California state nursing license required; pending licenses are accepted but must be active before the start date.
• Must be a first-time traveler and agree to undergo screening as necessary.
• Availability to work with no more than 7 days of restrictions on Time Off (RTO).
