Case Manager

Job Description

We are seeking a qualified Case Manager to join our team. The ideal candidate will have a strong background in acute care settings and a demonstrated ability in case management and discharge planning.

Key Responsibilities

• Coordinate patient care and discharge planning in an acute care environment.
• Maintain a manageable patient caseload adhering to established ratios: 1 Case Manager to 25 patients on weekdays and 1 Case Manager to 45 patients on weekends.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Participate in departmental weekend coverage as required (every other weekend).

Qualifications

• Minimum of 2 years of experience in Acute Care Case Management or Discharge Planning.
• Relevant hospital experience is required; behavioral health experience does not apply.
• Current California state license required (pending licenses accepted, must be licensed by start date).
• Maximum of 7 days of requested time off (RTO) allowed.
