Licensed Practical Nurse (LPN) – Inpatient Rehabilitation

Join our dedicated team as a Licensed Practical Nurse (LPN) in our Inpatient Rehabilitation setting, where you will provide high-quality care to adults recovering from various conditions. Our focus is on maximizing health, function, and quality of life for our patients.

Position Overview

• Department: Rehabilitation
• Role: LPN
• Shift: 12-hour shifts from 6:30 PM to 7:00 AM
• Weekend Requirements: Every other weekend and holiday rotations
• Call Requirements: Yes

Key Responsibilities

• Provide direct care to patients with high acuity needs.
• Assist nursing staff and healthcare team members in providing optimal patient care.
• Respond promptly to call lights and support patient safety initiatives.
• Utilize mobility equipment and assist patients in their rehabilitation activities.
• Maintain accurate patient records through electronic health record (EHR) systems.

Qualifications

• Valid LPN license.
• Minimum of 13 weeks of experience working with EPIC EHR.
• Inpatient rehabilitation or Med-Surg experience preferred.
• Certifications: Current BLS certification.
• At least 1 year of nursing experience in a skilled nursing or similar environment.

Unit Information

• Number of Beds: 49
• Patient Ratios: 1:7
• Equipment: Mobility aids, vital machines, EKG machines, computers for charting.

Additional Information

• Mandatory 30-minute breaks for 12-hour shifts; 15-minute paid breaks are included.
• Team collaboration is key; nurses are expected to assist C.N.As and other staff as required.
• Attendance is strictly monitored as per internal policies.
