Job Description

Licensed Practical Nurse (LPN) – Inpatient Rehabilitation

Position Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in an acute inpatient rehabilitation setting. The ideal candidate will possess at least 13 weeks of experience using EPIC and have previous experience in inpatient rehabilitation or medical-surgical nursing.

Responsibilities

Provide direct patient care in an inpatient rehabilitation environment with a focus on maximizing patient health, function, and quality of life.
Assist patients recovering from stroke, brain and spinal cord injuries, amputations, and complex orthopedic injuries.
Collaborate with a multidisciplinary team to develop and implement individualized care plans.
Monitor and assess patient conditions, ensuring safety and comfort through effective communication and interaction.
Utilize mobility equipment and assistive devices to facilitate patient mobility and rehabilitation efforts.
Manage medication administration, wound care, and other nursing duties as required.
Support nursing assistants and staff in maintaining a safe and efficient environment, responding to call lights and assisting with patient needs.

Qualifications

Minimum of 1+ years nursing experience in skilled nursing or similar 24/7 care environment.
Must have a current Basic Life Support (BLS) certification.
Experience in medical-surgical or inpatient rehabilitation is preferred due to the high acuity of patient cases.
Ability to work 12-hour shifts, including nights, weekends, and holiday rotations.
Willingness to adhere to attendance policies and clock out for designated breaks during shifts.

Patient Ratios: 1:7
Number of Beds: 49
Utilized Equipment: Mobility lifts, EKG machines, vital sign monitors, and electronic health record systems.
Patient Population: Adults requiring specialized inpatient rehabilitation services.
