We are seeking a dedicated Licensed Practical Nurse (LPN) to join our rehabilitation team. In this role, you will provide essential patient care, support recovery for individuals with various conditions, and contribute to their rehabilitation journey.

• Number of Beds: 36
• Patient Ratios: 1:7, can go up to 1:8
• Electronic Health Record (EHR): Wellsky
• Patient Population: Geriatric, Adult

Responsibilities

• Deliver high-quality nursing care to patients in a rehabilitation setting.
• Assist with daily living activities and promote patient independence.
• Monitor patient progress and report any changes to the healthcare team.
• Collaborate with multidisciplinary teams to develop and implement care plans.
• Maintain accurate patient records and documentation.

Qualifications

• Education: Completion of an accredited LPN program.
• Experience: 2 years of nursing experience preferred.
• Skills: Strong interpersonal and communication skills.
• Personality Fit: Compassionate, patient-focused, and a team player.
