Licensed Practical Nurse (LPN)

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in an inpatient rehabilitation setting. This role involves providing comprehensive nursing care to patients recovering from complex medical conditions.

Position Overview

The LPN will serve a critical role in a 50-bed acute inpatient rehabilitation facility, focusing on patient populations with high acuity needs. Responsibilities include collaborating with a multidisciplinary team to deliver quality patient care in a dynamic environment.

Key Responsibilities

• Provide nursing care in adherence to established protocols and standards.
• Assist in the development and implementation of individualized care plans.
• Monitor and assess patient conditions, reporting any changes to the nursing team.
• Administer medications and treatments as prescribed.
• Support patients in daily activities, promoting independence and mobility.
• Coordinate with healthcare professionals to ensure holistic patient care.
• Participate in maintaining a safe and clean environment.

Qualifications

• Minimum of 13 weeks of experience with the EPIC electronic health record system.
• Must possess inpatient rehabilitation or medical-surgical experience.
• At least 1 year of nursing experience in a skilled nursing or 24/7 skilled care environment.
• Current Basic Life Support (BLS) certification.
• Ability to work various shifts, including night shifts, with rotation on weekends and holidays.
• Excellent communication and interpersonal skills.
• Commitment to patient safety and quality care.

Unit Information

• Number of Beds: 49
• Patient Ratios: 1:7
• Essential Equipment: Mobility aids, vital signs monitoring machines, EKG machines, electronic charting systems.

Additional Information

All staff are expected to actively engage in teamwork, assisting Certified Nursing Assistants (CNAs) and responding to patient needs promptly. Attendance and punctuality are critical, adhering to the internal attendance policy.
