LPN (NIGHTS) – Inpatient Rehab

Position Overview

We are seeking dedicated LPNs to join our team in an inpatient rehabilitation setting. The ideal candidate will have a strong background in nursing, particularly in rehabilitation or medical-surgical environments, to provide high-quality care for patients referred from trauma centers.

Key Responsibilities

• Provide direct patient care in an inpatient rehabilitation setting.
• Assist patients with activities of daily living and support their rehabilitation goals.
• Collaborate with an interdisciplinary team to create and implement patient care plans.
• Monitor and document patient progress, vital signs, and overall condition in accordance with protocol.
• Support nursing assistants and respond to patient needs effectively to ensure safety and care quality.
• Participate in weekend and holiday rotations as required.
• Adhere to attendance and punctuality standards.

Qualifications

• Minimum of 1 year of nursing experience in skilled nursing or a 24/7 healthcare environment.
• Prior experience in inpatient rehabilitation or medical-surgical nursing is preferred.
• Certification in Basic Life Support (BLS) is required.
• Must have at least 13 weeks of experience using EPIC electronic health record system.

Unit Information

• Bed Capacity: 50
• Patient Ratios: 1 nurse per 7 patients
• Common Equipment: Mobility aids, vital signs monitors, EKG machines, and electronic charting systems.
• Patient Population: Adults requiring acute inpatient rehabilitation services with diverse needs.

Working Conditions

This position requires 12-hour shifts from 6:30 PM to 7:00 AM, with a rotating schedule including weekends and holidays. Staff members will have mandatory breaks and must adhere to timekeeping policies.
