LPN (NIGHTS) – Rehab

Position Information:

• Role: LPN
• Specialty: Inpatient Rehabilitation
• Shift: 12-hour shifts, 6:30 PM to 7:00 AM
• Weekend Requirements: Every other weekend and holiday rotations
• Call Requirements: Yes
• Required Certifications: BLS
• Years of Experience: Minimum of 1 year in a skilled nursing or other 24/7 skilled care environment
• Preferred Experience: Medical Surgical or inpatient rehabilitation, particularly with high acuity patients
• Float Requirements: No current float requirement, may be required to float to nearby facilities in the future

Attendance and Scheduling Policies:

• All staff are required to adhere to the internal attendance and tardiness policy.
• Mandatory 30-minute breaks unless otherwise approved by a supervisor. Staff must clock out during this time.
• Staff will be allocated two 15-minute paid breaks during a 12-hour shift.
• Staff are not permitted to extend their shift without approval from the house supervisor.

Unit Information:

• Number of Beds: 49
• Patient Ratios: 1:7
• Equipment: Mobility aids including ceiling lifts, Sara Stedy, Sara Plus, slide boards, vital machines, EKG machines, blood glucose monitors, and computers for charting.
• EHR: EPIC
• Patient Population: Adults requiring inpatient rehabilitation services with high acuity needs.

Responsibilities:

As an LPN in this inpatient rehabilitation setting, you will be involved in providing high-quality patient care, assisting with daily activities, and supporting nursing staff. You may also be required to assist Certified Nursing Assistants (C.N.As) and respond to call lights to ensure patient safety and comfort.

Qualifications:

The ideal candidate will possess a valid nursing license, hold BLS certification, and have a solid background in skilled nursing or related care environments. Previous experience in a medical surgical or inpatient rehabilitation setting is preferred due to the nature of patient acuity.
