We are seeking a dedicated Licensed Practical Nurse (LPN) to join our rehabilitation team. This position provides essential care to patients who are recovering from severe injuries or illnesses, including stroke, brain injury, spinal cord injury, and orthopedic issues.

Position Overview

• Role: LPN
• Department: Chronic 1/106
• Shift: 6:00 AM – 6:00 PM
• Weekly Guaranteed Hours: 36
• Patient Ratios: 1:7
• Age Range of Patients: 16 years and older

Essential Responsibilities

• Deliver comprehensive nursing care tailored to individual patient needs.
• Monitor patient progress and adjust care plans as necessary.
• Collaborate with an interdisciplinary team to ensure effective rehabilitation processes.
• Document patient care and maintain accurate medical records using the EHR system.

Qualifications

• Required Certifications: BLS Certification
• Years of Experience: Minimum of 2 years in a clinical setting.
• Preferred Experience: Previous experience in rehabilitation settings.

• Scrub Color: Eggplant for LPN
• Time to Report: 6:45 AM
• Who to Ask For Upon Arrival: Charge Nurse
• Parking: Available in any non-visitor marked space in the parking lot.
