LTC/SNF LPN

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for our Long-Term Care/Skilled Nursing Facility. This role involves providing exceptional patient care to elderly individuals with multiple diagnoses.

Job Details

• Unit Type: LTC/SNF
• Shift Length: 12 hours
• Guaranteed Hours: 36 hours per week
• Years of Experience Required: 1 year
• Certifications: BLS Certification required
• Shifts: 6:00 AM – 6:30 PM; every other weekend and holiday

Facility Information

• Bed Capacity: Current census is 42 beds
• Patient Ratios: Varies; days shift is 1:21, at most 1:42
• Electronic Medical Records: PCC system
• Weekend Requirement: Every other weekend
• Holiday Requirement: Every other holiday
• Orientation: 1 day of location-specific orientation followed by up to 3 days on the floor with staff
• Common Patient Diagnoses: Primarily elderly patients with multiple diagnoses

Responsibilities

The LPN will be responsible for providing high-quality nursing care to residents, monitoring their health status, administering medications, and collaborating with other healthcare professionals to ensure the best outcomes for patients.

Additional Notes

• Local travel may be required within 80 miles.
• No additional charge differentials or call back duties apply.
• Resignation with less than 14 days’ notice will incur a fee.
