LTC/SNF LPN

Job Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care/Skilled Nursing Facility (LTC/SNF) team. The ideal candidate will demonstrate clinical expertise, along with a strong commitment to providing exceptional care to residents.

Shift Information

• Day and Evening Shifts available
• Shift Length: 8 hours
• Schedule includes every other weekend and holiday

Responsibilities

• Provide direct patient care and assist in the development of nursing care plans.
• Administer medications and treatments as prescribed by the healthcare provider.
• Monitor and document patient progress and any changes in condition.
• Communicate with patients and their families about health status and care plans.
• Participate in team meetings and contribute to care planning.

Qualifications

• Must have at least 2 years of experience in a nursing role within a LTC/SNF environment.
• Current BLS Certification required.
• Strong interpersonal and communication skills.
• Ability to work effectively as part of a team and independently.

Additional Information
