LPN (NIGHTS) – Rehab

Position Overview

This position involves providing nursing care in an inpatient rehabilitation setting. The role primarily focuses on maximizing patients’ health, function, and quality of life through comprehensive rehabilitation programs.

Key Responsibilities

• Deliver patient care in accordance with established protocols and standards.
• Assist patients with activities of daily living to promote independence.
• Collaborate with a multidisciplinary team to develop and implement individualized care plans.
• Monitor and document patient progress, responding appropriately to changes in condition.
• Ensure that patient safety and comfort are prioritized by maintaining a clean and safe environment.
• Participate in patient education and discharge planning efforts.
• Assist nursing aides and help with call lights for patient safety and support.

Qualifications

• Must possess a valid LPN license.
• MUST HAVE A MINIMUM OF 13 WEEKS OF EPIC EXPERIENCE.
• Minimum of 1 year of nursing experience in a skilled nursing or other 24/7 care environment required.
• Experience in inpatient rehabilitation or Med Surg preferred.
• Current BLS certification is required.
• Ability to work every other weekend and participate in holiday rotations.

Unit Information

• Number of Beds: 50
• Patient Ratios: 1:7
• High acuity unit, functioning similarly to a medical surgical unit.
• Use of mobility equipment, including ceiling lifts and vital machines.
• EHR system: EPIC

Orientation Dates

• October 20th
• November 17th
• December 1st

Additional Information

Attendance and punctuality are critical for providing optimal patient care. Scheduling includes mandatory breaks, with provisions for supervision approval for extended hours. Staff may be required to float to a nearby facility within 20 minutes if needed in the future.
