Physical Therapist

We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will provide exceptional care in skilled nursing facilities and home health settings. This position is suitable for new graduates, and mentorship will be provided by experienced staff.

Key Responsibilities:

• Conduct evaluations and assessments of patient needs.
• Develop and implement individualized therapy plans.
• Supervise and mentor Physical Therapist Assistants.
• Maintain accurate patient records and documentation.
• Collaborate with interdisciplinary teams to enhance patient outcomes.

Qualifications:

• Master’s or Doctorate degree in Physical Therapy from an accredited program.
• Valid state licensure or eligibility for licensure.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team environment.
• Commitment to delivering high-quality patient care.

Position Details:

• Settings: Skilled nursing facilities and home health care.
• Supervising: 2 PTAs.
• Software experience: SNF Net Health and HH HCHB preferred.
• Productivity expectation: 80% in SNF; points system applicable in home health.
• Therapy Supervisor available on-site.
• Scheduling: Varies; primarily day shifts.
• Guaranteed hours: 40 hours per week.
• Targeted start date: As soon as possible.
