Case Manager

We are seeking a qualified Case Manager to join our team. This position requires acute care experience and a strong background in discharge planning and care coordination.

Key Responsibilities:

• Provide comprehensive case management services across various units.
• Assist patients in transition to post-acute care through effective discharge planning.
• Coordinate care with interdisciplinary teams to ensure optimal patient outcomes.
• Conduct assessments and develop care plans tailored to individual patient needs.
• Manage cases across multiple areas, ensuring compliance with regulations and standards.

Qualifications:

• Must have at least 1 year of acute care experience.
• Previous travel experience is required.
• Experience with Epic EMR is mandatory.
• AIDA EMR experience is preferred.
• Certifications required: BLS and active WA nursing license.
• Experience in Med Surg, LLOS, and ED is strongly preferred.

Work Schedule:

• Shift: 5×8 Days (07:30 – 16:00)
• Weekends required: Yes, every other weekend.
• Guaranteed hours: 72 hours per 2-week period.

Additional Information:

• Floating between like units is expected.
• This position requires working across multiple locations within the region.
• All requests for time off (RTO) must be submitted at the time of application.
• Locals will not be accepted for this position.
• Parker Cost: Facility paid.
