Job Summary

The Program of All-Inclusive Care for the Elderly (PACE) provides various services designed to allow older individuals to continue living in their communities instead of nursing homes. This position is part of an interdisciplinary care team that coordinates and delivers preventive, primary, acute, and long-term care services.

Primary Purpose

The Clinical Care Registered Nurse (CCRN) will provide direct nursing care to participants, working independently to handle complex clinical situations while collaborating with both licensed and non-licensed personnel.

Duties and Responsibilities

• Monitor acute and chronic medical conditions following participants’ care goals.
• Perform wound care and treatments as prescribed in care plans.
• Administer medications, immunizations, and other care as instructed.
• Conduct assessments and actively participate in developing care plans.
• Make regular home visits to deliver care, including wound care, lab draws, and nurse assessments.
• Address on-call needs and assess participants’ immediate requirements.
• Work beyond normal hours, including weekends and holidays, as necessary.
• Complete all required educational and documentation requirements in a timely manner.
• Document in participants’ medical records according to policy.
• Collaborate with staff and participants’ families, utilizing a participant-centered care model.
• Keep leadership informed of any issues or problems.
• Perform additional duties as assigned.

Qualifications

• Bachelor’s degree in Nursing preferred; Associate degree required.
• Nursing diploma or degree from an accredited School of Nursing required.
• Proficiency with Microsoft Word and Outlook is necessary.
• Ability to exercise discretion and confidentiality effectively.
• Strong written and verbal communication skills.
• Minimum of one year of experience with the frail elderly or long-term care population.
• Preferred one year of home care experience.
• Preferred one year of wound care experience.

Specific Requirements

• Current CPR certification by AHA or willing to obtain as offered.

• Immunizations and vaccinations must be current or medical documentation must be provided for waivers.
• Reliable transportation required; must have a valid driver’s license and insurance.

Work Environment

• Visible presence while working in the center with frequent interruptions.
• Communication with staff, participants, caregivers, and other organizations as necessary.
• May be required to work beyond normal hours during emergencies or severe weather.
• Potential exposure to infectious waste, diseases, and hazardous materials.
• Callback during emergency conditions may be required.
• Driving in various weather conditions is a necessity.

Physical Demands

• Ability to cope with the mental and emotional stress of the role.
• Function independently and demonstrate flexibility and integrity.
• Ability to work effectively with participants, staff, and support agencies.
• Required to push, pull, move, sit or stand for extended periods.
• Lifting up to twenty pounds as needed.

Minimum Working Requirements

• Must be at least 18 years of age.
• Must be fully vaccinated or willing to be vaccinated against specified communicable diseases.

Holiday Coverage Required
