Licensed Practical Nurse (LPN)

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join our team in a rehabilitation setting aimed at providing exceptional care for patients recovering from various conditions.

Position Overview

This position requires a flexible and collaborative approach to nursing with a focus on enhancing each patient’s functional abilities and quality of life. The candidate will work closely with a team of healthcare professionals to deliver comprehensive patient care.

Key Responsibilities

• Provide direct patient care in a rehabilitation environment.
• Monitor patient progress and document changes in condition.
• Collaborate with healthcare team members to plan and implement patient treatment and rehabilitation programs.
• Assist patients with daily living activities, mobility, and functional tasks.
• Educate patients and their families regarding health maintenance and disease prevention.

Qualifications

• Valid LPN license in the state.
• BLS (CPR) Certification is required.
• Minimum of 2 years of nursing experience in a rehabilitation or medical/surgical setting.
• Familiarity with electronic health record (EHR) systems; experience with Epic is a plus.
• Preferred certification as a Certified Rehabilitation Registered Nurse (CRRN).

Work Schedule

This position entails a shift from 6:30 AM to 7 PM, with requirements for weekend work every other weekend.

Patient Care Environment

Our facility features 81 private rooms, with a patient ratio of approximately 1:6-7 during the day and 1:8 at night. Each nurse is supported by a dedicated technician to ensure quality care is maintained.

Onboarding Details

Upon successful selection, candidates are required to wear black scrubs and report to the Charge Nurse upon arrival, with parking available in the designated garage areas.
